Stated action · August 2018
“It is accepted that there may be missed opportunities to learn from incidents where an offender subject of multi-agency management causes death or serious injury and a multi-agency review does not take place. At present there is no national police guidance in relation to the internal reviews of incidents where an offender, subject of supervision by the integrated offender management (IOM) team, is involved in a further serious offence such as homicide.”
