29 pairs where a later report names an earlier report to the same organisation, at least six months apart. Blind test: those sentences were removed before the system ran. Each system shows the same number of links (1217); no threshold was tuned on these pairs.
Post-hoc: the report route was added after seeing that most misses never reached the judge, so its result is not an independent test. It finds 15 of 29 (52%): 0 more than before, 0 fewer. It shows 1360 links; at that larger budget the baselines find tf-idf only 45%, embeddings only 52%. No other test set of coroner-acknowledged pairs exists, so it has not been validated on held-out pairs.
With only 29 pairs the ranges overlap: the ordering is consistent, but the gap is not yet statistically settled.
Precision (the share of shown links a person would accept) needs hand labels. Status: pending: no labels filled in yet.
The Nemotron system missed 14 of the 29 pairs: 9 never reached the judge and 5 the judge said it was a different hazard. Most misses happen before the judge: no pair of concerns passed the candidate filter, which caps how many pairs each organisation sends to the judge. Large national bodies, with hundreds of earlier reports, are hit hardest. When the judge did see a pair and said no, it called it a related but different hazard.
