The coroner's concerns

  1. 1. failure to diagnose breach presentation

“At 02.40 on 26 August 2014 whilst in labour Mrs [name redacted] was subjected to an abdominal and a vaginal examination. The results of the examination did not correlate. Despite this fact no further investigations, to include an ultrasound scan, were undertaken to confirm [the deceased]'s presentation. This resulted in Mrs [name redacted] being considered a low risk delivery and a breach position not being diagnosed. In turn this prevented appropriate planning tegarding a preferred method of delivery.”
