The coroner's concerns
1. lack of multi-agency meeting policy for IPP offenders
“where an IPP offender with complex needs is released and is not subject to MAPPA, but is subject to multi-agency intervention co- ordinated by the National Probation Service, there is no policy/procedure/guidance encouraging consideration to be given to the issues of whether and when to hold multi- agency meetings for the purposes of sharing information about the offender. The need for consideration to be given to holding such meetings is particularly acute where one of the organisations responsible”
