1. inadequate documentation of newborn care
“limited documented information available to treating clinicians following [the deceased]'s birth as to the events and treatment which had been provided to her”
2. retrospective note taking based on assumption
“some of the notes completed retrospectively were based on assumption rather than first hand knowledge”
